The Inpatient Pharmacy Department at Kaweah Health Medical Center provides progressive pharmacy services across three campuses and includes a staff of 160+ with over 45 clinical pharmacists. The Inpatient Pharmacy operates 24/7 pharmacy services. A full array of technology is available to assist with distribution of medications to our patients including automated inventory management, automated dispensing cabinets, bar code technology, automated TPN and workflow management systems.
Clinical Inpatient Pharmacy services utilize an integrated decentralized staffing model. Pharmacists are stationed on nursing units participating in order verification, provision of drug information, and clinical services. Clinical services include pharmacokinetics, anticoagulation management, and automatic intravenous to oral switches, automatic renal dose adjustments and automatic therapeutic interchanges. Specialty pharmacy services include Anemia Management, Anticoagulation, Critical Care, Emergency Medicine, Formulary Management, Infectious Diseases, Oncology, Pain Management, and Pediatrics.
Position Description:
Clinical Pharmacist –Acute Care – Variable Shift:
Clinical Pharmacist assigned to rotate on acute care general medicine and surgical units supporting medication related distributive, clinical, and educational functions including precepting of pharmacy students and PGY1 residents.
Responsibilities include:
Serves as the clinical pharmacy expert on assigned medical/surgical unit(s) during scheduled shift
Participates in the care of and reviews/verifies medication orders for patients in this setting
Organizes treatment and monitoring plans for targeted medications by protocol including pharmacokinetic consults, warfarin consults, IV to PO interchanges and automatic renal dose adjustments
Participates in emergency responses (e.g. Code Blue, Rapid Response) for assigned units
Assures compounded medications meet standards for quality when performing or supervising compounding
Documents clinical interventions to measure the clinical & economic effectiveness of clinical pharmacy services
Teaching/precepting of PGY1 residents and pharmacy student learners
Opportunity to contribute to related patient care projects, policy & procedure development, order sets, formulary reviews and institutional committees

License\Certification
Requires a current California Registered Pharmacist license.
Education\Qualifications
Doctor of Pharmacy (Pharm. D.) degree required.
Completion of PGY 1 residency or at least two years acute inpatient hospital pharmacist experience
